Theophylline anhydrous
Indications/Uses
Relief & prophylaxis of reversible bronchospasm associated w/ bronchial asthma, bronchitis, emphysema & related conditions.
Dosage/Direction for Use
Adult 1 tab 12 hrly. May be gradually increased or decreased by ½ tab if sufficient effect is not achieved.
Administration
Should be taken with food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to theophylline or to xanthines.
Special Precautions
Monitor plasma levels. Acute asthma. Patients w/ reduced thyroid function, CHF, acute pulmonary oedema, COPD, severe hypoxia, pneumonia, acute & febrile episodes, & acute viral infection; cardiac arrhythmias, CAD, unstable angina, cardiomyopathy & severe HTN; peptic ulcer or gastro-oesophageal reflux. Smoking. Do not administer concurrently w/ other xanthines. Concomitant use w/ sympathomimetic agents. Xanthine-containing beverages eg, tea, coffee, cola, cocoa. Pregnancy & lactation. Childn <2 yr. Sick patients & elderly.
Adverse Reactions
Gastric irritation, nausea, vomiting, anorexia, epigastric pain, reactivation of peptic ulcer, gastro-oesophageal reflux, haematemesis, tachycardia, palpitation, headache, CNS stimulation, reflex hyperexcitability, insomnia & tremor.
Drug Interactions
Decreased hepatic clearance w/ cimetidine, high dose allopurinol, propranolol, macrolide antibiotics (eg, erythromycin, clarithromycin), quinolone antibiotics (eg, ciprofloxacin & enoxacin), alcohol, OCs, mexiletine, tacrine, thiabendazole, disulfiram, interferon α & verapamil. May lower serum conc w/ tobacco or marijuana smoking, phenobarb, phenytoin, carbamazepine & rifampicin. Reduced plasma levels w/ drugs containing St. John's wort (Hypericum perforatum). Ventricular arrhythmias may occur w/ halothane. May lower seizure threshold w/ ketamine. May enhance renal clearance of lithium. Synergism w/ adrenaline & other sympathomimetic amines. Concomitant use w/ methylxanthines, ranitidine, diltiazem, nifedipine, INH, furosemide, flu vaccine & corticosteroids.
